Description

Mourani et al identified risk factors for readmission to the intensive care unit (ICU) for a pediatric patient with a history of preterm birth and respiratory failure. These can help to identify a patient who may benefit from more aggressive management. The authors are from the University of Colorado, Children’s Hospital Colorado, University of Pittsburgh, and the University of Alabama.


 

Patient selection: preterm delivery with respiratory failure requiring mechanical ventilation (with birth weights from 500 to 1,250 grams)

 

Outcome: ICU readmission during childhood

 

Risk factors for ICU readmission during childhood (from univariate analysis):

(1) male gender (odds ratio 2.0)

(2) intracranial hemorrhage Grade 3 or Grade 4 (odds ratio 2.1)

(3) duration of birth hospitalization (odds ratio 1.01 per day)

(4) duration of oxygen therapy during birth hospitalization (odds ratio 1.01 per day)

Over half of the children had one or more readmissions to the hospital.

About a quarter of the children had multiple readmissions to the hospital.

About 20% of the children were readmitted to the ICU.
