Pregabalin (Lyrica) has significant renal excretion and its dose should be adjusted in a patient with renal impairment.
Parameters:
(1) estimated creatinine clearance in mL per min
(2) daily dose of pregabalin in mg per day
(3) hemodialysis
Standard daily doses:
(1) 150 mg/day
(2) 300 mg/day
(3) 450 mg/day
(4) 600 mg/day
