Description

Kudo et al identified predictors for delayed encephalopathy in a patient admitted for acute carbon monoxide poisoning. These can help to identify a patient who may need closer monitoring following hospital discharge. The authors are from Iwate Medical University in Japan.


 

Patient selection: admitted for acute carbon monoxide poisoning

 

Direct measures of carbon monoxide toxicity:

(1) unrousable despite noxious stimuli (Japanese Coma Score or JCS >= 100) on admission

(2) brain CT or MRI changes of hypoxic encephalopathy

(3) elevated serum enzyme concentrations (CK, LDH)

(4) psychiatric disability by the Global Assessment Scale (GAS)

 

Indirect measures of carbon monoxide toxicity:

(1) high number of hyperbaric oxygen therapy sessions (> 20 sessions)

(2) prolonged hospitalization (> 60 days)
