Description

Xu et al identified risk factors for rapidly-progressive interstitial lung disease (ILD) in a patient with clinically amyopathic dermatomyositis (CADM). This can help to identify a patient who may require aggressive management. The authors are from China Medical University in Shenyang.


 

Patient selection: clinically amyopathic dermatomyositis (CADM)

 

Patients with antibodies to melanoma differentiation-associated protein-5 (MDA-5) is associated with:

(1) severe cutaneous vasculopathy

(2) interstitial lung disease (ILD)

 

Risk factors for rapidly-progressive ILD:

(1) antibodies to MDA-5

(2) C-reactive protein (CRP) > 50 µg/L

(3) absolute lymphocyte count < 500 per microliter

 

Additional risk factors:

(1) serum ferritin > 2,000 ng/mL

(2) skin ulcerations
