Description

A patient with Schistosoma mansoni may develop a colonic stricture that is potentially reversible.

Clinical features:

(1) The patient is infected with Schistosoma mansoni.

(2) A granulomatous reaction develops to schistosomal eggs deposited in the submucosa of the colon.

(3) Fibrosis may develop which can result in a colonic stricture.

(4) A few patients may develop bowel obstruction.

The strictures can revert if the patient is treated with antischistosomal drugs (such as niridazole).

Severe strictures with extensive fibrosis may persist despite treatment. The patient may require surgery of obstruction occurs.

The differential diagnosis includes colonic carcinoma.
