Description

Partial priapism is a rare condition affecting the proximal penis.

Clinical features:

(1) distention of the proximal penis, sometimes presenting as a perineal mass

(2) flaccidity of the distal penis

(3) usually associated with perineal pain

The most common cause is segmental thrombosis of the corpora cavernosa. Following thrombosis there may be persistence if there is fibrosis in the corpora cavernosa with vascular obstruction.

Some authors include hematoma as a cause.

Preceding events may include:

(1) trauma

(2) bicycle riding

(3) sexual intercourse

(4) hematologic disorders

(5) alpha-blocker therapy

(6) substance abuse

CT or MRI imaging can help to define the nature and extent of the disorder.
