Trimethoprim-sulfamethoxazole (TMP-SMX) is the first-line therapy for the oral eradication phase of meliodosis. If problems arise then amoxicillin-clavulanate may be used.
Indications for amoxicillin-clavulanate therapy:
(1) resistance to TMP-SMX
(2) unacceptable side effects to TMP-SMX
Duration of therapy: 3 to 6 months
Dosing frequency: 3 times a day
Route: oral
Dose of amoxicillin: 20 mg per kilogram body weight (ideal)
Dose of clavulanate: 5 mg per kilogram body weight (ideal)
