Description

An index for the early prognosis after acute myocardial infarction was developed using noninvasive parameters gathered within 24 hours from the onset of symptoms. This helps identify those patients who will die in hospital of left ventricular power failure.

discriminant function =

= (3.9 * (age in years)) - (1.3 * (systolic blood pressure in mm Hg)) - (1.4 * (left ventricular ejection time in milliseconds)) + (25.3 * (pulmonary venous congestion score)) + 775.3

where:

• The pulmonary venous congestion score is a value from 0 to 3 based on the degree of pulmonary congestion seen on chest X-ray.

Interpretation

Discriminant Function Score

Survivors

Nonsurvivors due to Power Failure

> 550

16%

87%

<= 550

84%

13%

Limitations:

• This index was developed over 20 years ago, before many current diagnostic and therapeutic modalities were available.

• In the study, 160 people survived, 30 people died of power failure and 5 people died of other causes. Using the percentages shown, about 26 survivors and 26 nonsurvivors had scores > 550.
