Description

Residents living in the high Andes develop a chronic sensation of burning or tingling in the hands and feet. The neurological findings are similar to the hypoxic neuropathy occurring in patients with COPD.


 

Features:

(1) resident at high altitude

(2) neurologic signs and symptoms

(3) centrifugal resolution of symptoms if the patient stays at a low altitude

(4) exclusion of other causes for peripheral neuropathy (toxic, nutritional, diabetes, etc.)

 

Most common neurologic signs and symptoms:

(1) migraine-like headache

(2) burning or tingling paresthesias in feet and/or hands

 

Variable neurologic signs and symptoms:

(1) dizziness

(2) depression or loss of distal reflexes

(3) impaired distal sensation in hands and/or feet

(4) ataxia on clinical examination

 

Biopsy of a peripheral nerve shows a mild demyelinating neuropathy.
