Filipovich et al developed criteria for the severity of chronic graft-versus-host disease (GVHD) for the National Institutes of Health. This includes assessment of overall performance score and a summary of organ grades.
Patient selection: chronic GVHD
Grading overall performance status may be based on:
(1) ECOG
(2) Karnofsky performance status (KPS)
(3) Lansky Play Performance Status for children

The final step is to tally the number of Grade 1, Grade 2 and Grade 3 organ involvements (including performance status).
Systemic immunosuppressive therapy should be considered if either:
(1) number of organs with Grade 2 or 3 involvement >= 1, OR
(2) number of organs with Grade 1, 2 or 3 involvement >= 3
