Description

Assessing myeloma patients for performance status, hemoglobin, serum calcium and BUN allowed identification of those patients with good or bad survival.

Performance status (ECOG):

(0): asymptomatic with no clinical evidence of disease (Karnofsky index 100)

(1): symptomatic but able to continue normal work and activities (Karnofsky index 80-90)

(2): too ill to work but able to care for self, with less than 50% of time in bed (Karnofsky index 60-70)

(3): requires considerable assistance and frequent medical care, with more than 50% of time spent in bed (Karnofsky index 50)

(4): severely disabled, requires hospitalization and active supportive treatment, with > 95% of time spent in bed (bedridden; Karnofsky index < 50)

Risk factors associated with worse survival:

(1) hemoglobin < 9 g/dL

(2) blood urea nitrogen > 30 mg/dL

(3) serum calcium is > 12 mg/dL

(4) performance status 2, 3 or 4

where:

• conversion of BUN from mg/dL to mmol/L is 0.357; from mmol/L to mg/dL is 2.80112

• conversion of calcium from mg/dL to mmol/L is 0.25; from mmol/L to mg/dL is 4.0
