Description

Therapy with mycophenolate mofetil has been associated with colitis.


 

Patient selection: transplant patient being treated with mycophenolate mofetil

 

Presentation: diarrhea with weight loss and variable decrease in renal function

 

Diagnosis:

(1) The patient has the onset of colitis after starting therapy with mycophenolate mofetil.

(2) The colitis may be segmental or a pancolitis.

(3) A colon biopsy shows a colitis resembling ischemic colitis or graft-vs-host disease.

(4) Discontinuing the mycophenolate therapy is followed by resolution of the colitis.

(5) Other causes of colitis (ischemia, GVHD, CMV, etc) are excluded.
