There is considerable overlap between “metastatic” Crohn’s disease and the extraintestinal manifestiona.
Criteria for a metastatic manifestation of Crohn’s disease:
(1) not contiguous with the gastrointestinal tract
(2) biopsy shows granulomatous inflammation
(3) exclusion of other causes
(4) improvement after starting therapy targeting Crohn’s disease
The onset of a metastatic lesion may precede or follow the onset of intestinal disease.
The problem with the word “metastatic” is that it suggests that something from an affected intestine migrates to a remote site similar to circulating tumor cells.
