Description

Unroe et al listed risk factors for discrepancies in medication reconciliation performed at hospital admission. The authors are from Duke University, Durham VA Medical Center, University of North Carolina at Chapel Hill and Monroe Carell Jr Children’s Hospital at Vanderbilt in Nashville.


Patient selection: patient being admitted to the hospital

 

Outcome: discrepancy during drug reconciliation including unintended drug omission, error in name or error in dose

 

In the multivariate analysis the key risk factor is receipt of one or more high risk medications.

 

Other risk factors from the multivariate analysis:

(1) older age (odds ratio 1.16 per 5 year increment, with risk in the univariate analysis when age >= 65 years)

(2) admission to a surgery service (vs general medicine or cardiology service)

 

Additional factors affecting discrepancies:

(1) total number of doctors being seen

(2) number of comorbid conditions

(3) total number of drugs being taken

(4) accuracy of the available information about the home drug regimen
