Description

Bekhof et al developed a nomogram to help identify a preterm infant with late-onset sepsis. The authors are from Isala Klinieken, University Medical Center Utrecht, and the Academic Medical Center in Amsterdam, The Netherlands.

Patient selection: post-conception age < 34 weeks and > 72 hour postnatal age, not on antibiotics for the last 24 hours

End-point: clinical and/or blood culture-proven sepsis

Parameters:

(1) renal insufficiency

(2) grey skin

(3) central venous line

(4) capillary refill time in seconds

= SUM(points for all 4 parameters)

Interpretation:

• minimum score: 0

• maximum score: 44

• The higher the score the greater the likelihood of late-onset sepsis.

probability of late-onset sepsis =

= (1.66 * (total score)) + 13
