Description

Ambalavanan et al graded disability in patients with hypoxic-ischemic encephalopathy based on clinical findings. This can help identify an infant who requires more aggressive management. The authors are from multiple universities in the United States participating in the National Institute of Child Health and Human Development Neonatal Research Network.


 

Parameters:

(1) gross motor function classification system (GMFCS) of Palisano et al (see 37.18.09)

(2) mental development index score from the Bayley scales of infant development II

(3) deafness with need for hearing aid

(4) blindness

(5) seizure disorder

 

Severe disability was defined as the presence of one or more of the following:

(1) mental development index score < 70

(2) GMFCS grade 3, 4 or 5 (unable to walk or crawl; needs support for sitting or requires adult assistance to move)

(3) requires hearing aid or totally deaf

(4) blind

 

Moderate disability - both of the following:

(1) mental development index score 70 - 84

(2) one or more of the following:

(2a) GMFCS grade 2 (unable to walk but can sit, pull to standing and crawl)

(2b) impaired hearing but does not require a hearing aid

(2c) persistent seizure disorder

 

Although not listed in the paper a mild disability might be defined as:

(1) mental development disability index score of 85 - 90+

(2) intermittent seizure disorder

(3) GMFCS grade 1 (walks independently but some gait abnormality)

(4) someone with some but not all of the criteria for moderate disability
