Infection with Blastomyces dermatitidis may be treated with amphotericin B if certain indications are present.
Indications for selecting amphotericin B to treat blastomycosis:
(1) central nervous infection
(2) immunocompromised patient
(3) pregnant patient
(4) inability to take azole drugs because of their adverse effects
(5) progressive disease while being treated with an adequate dose of azole drugs
(6) life-threatening infection (with adult respiratory distress syndrome, fulminant disease, etc)
