Chemotherapy with ifosfamide can result in an encephalopathy.
Ifosfamide is metabolized active metabolites that act as alkylating agents.
Risk factors for the encephalopathy:
(1) hypoalbuminemia
Manifestations:
(1) agitation, confusion and/or delirium
(2) psychosis
(3) cranial nerve palsy
(4) diplopia
(5) dysarthria
(6) aphasia
(7) incontinence
(8) muscle twitching
(9) seizures
(10) coma
The patient may improve:
(1) if ifosfamide is discontinued
(2) if methylene blue is infused
A patient who has experienced encephalopathy may be able to have future courses with ifosfamide without recurrence of the neurological findings.
Purpose: To evaluate a patient who is being treated with ifosfamide for evidence of ifosfamide encephalopathy.
