Description

Sometimes it may be necessary to infer the location of a prosthetic valve based on the appearance on chest X-rays. This may occur if the patient is unconscious and the medical record is not available.

Position in Cardiac Silhouette on PA View

Valve

central

aortic or mitral

inferior, to the right of the midline over the spine

tricuspid

just above the atrioventricular groove, close to origin of the great vessels

pulmonic

Features of aortic valve:

(1) superior and medial to mitral valve in PA view

(2) superior and anterior to the mitral valve in lateral view; centrally located

(3) struts point superiorly and to the patient's right

Features of mitral valve:

(1) inferior and lateral to aortic valve in PA view

(2) inferior and posterior to the aortic valve in lateral view

(3) struts point inferiorly and to the patient's left

Features of the pulmonic valve

(1) superior to the aortic valve in the PA view

(2) superior and anterior to the aortic valve in lateral view; abuts anterior cardiac silhouette

(3) struts point superiorly and to the patient's left
