A hypersecretory adenoma of the colon can result in the McKittrick-Wheelock Syndrome.
Features of McKittrick-Wheelock Syndome:
(1) The patient has a large ("giant") hypersecretory villous adenoma in the colon.
(2) The large volume of secreted fluid causes diarrhea, which may lead to volume depletion, hypokalemia and hyponatremia.
(3) The patient may develop acute renal failure.
Steven et al reported that indomethacin reduces the fluid loss by reducing release of prostaglandin E2.
The definitive management of the adenoma is surgical resection.
The adenoma may undergo:
(1) rectal prolapse
(2) intussusception
(3) bowel obstruction
(4) adenocarcinoma
