Description

Hashimoto's encephalopathy refers to a subacute encephalopathy developing in a patient with an autoimmune thyroiditis. The precise pathophysiology is unclear.


 

Synonym: steroid responsive encephalopathy associated with autoimmune thyroiditis (SREAT)

 

Clinical features:

(1) autoimmune thyroiditis with high titers of anti-thyroid autoantibodies (thyroid peroxidase antibody and/or thyroglobulin antibody)

(2) acute or subacute relapsing-remitting encephalopathy (may include altered level of consciousness, seizures, headache, dementia, psychomotor deficits, tremor, ataxia and other neurologic findings)

(3) rapid improvement to therapy with corticosteroids

(4) absence of another diagnosis that can explain the findings better

 

The patient may be euthyroid, hypothyroid or hyperthyroid.

 

The cerebrospinal fluid (CSF) shows a mild increase in total protein and/or a mild lymphocytosis.
