Held et al reported the hand eczema severity index (HECSI) for describing the severity of hand eczema. This can be used to monitor change over time and response to therapy. The authors are from Gentofte Hospital and the University of Copenhagen.
Patient selection: hand eczema
Sites assessed:
(1) fingertips
(2) fingers other than tips
(3) palms of the hands
(4) back of hands
(5) wrists
Both left and right sides are evaluated together for each measure.

subscore for each site =
= SUM(points for severity for the 6 changes) * (points for extent)
total score =
= SUM(subscores for each of the 5 sites)
Interpretation:
• minimum subscore for each site: 0
• maximum subscore for each site: 72
• maximum score for all 5 sites: 360
• The higher the total score the more severe the hand eczema.
Performance:
• Intraobserver and interobserver reliability was good.
