Qureshi developed a grading scheme for describing the severity of arterial occlusion seen on angiography in a patient with an ischemic stroke. The authors is from the University of Buffalo.
Parameters:
(1) site of occlusion
(2) collateral circulation
Grade 0 implies no occlusions are identified in any of the arteries.
Occlusion in the Middle Cerebral Artery |
Grade |
M3 segment |
1 |
M2 segment |
2 |
M1 segment, sparing of lenticulostriate arteries and/or leptomeningeal collaterals are visualized |
3A |
M1 segment, no sparing of lenticulostriate arteries and leptomeningeal collaterals are not visualized |
3B |
Occlusion in the Anterior Cerebral Artery |
Grade |
distal or A2 segment |
1 |
A1 and A2 segment |
2 |
Occlusion in the Basilar and/or Vertebral Artery |
Grade |
occlusion of 1 branch |
1 |
occlusion in >= 2 branches |
2 |
occlusion of basilar artery with antegrade filling, either by direct partial filling or filling via collaterals |
4A |
occlusion of basilar artery with retrograde filling, either by direct partial filling or filling via collaterals |
4B |
complete occlusion of the basilar artery without collateral filling |
5 |
Occlusion in the Internal Carotid Artery |
Grade |
present, with collaterals filling middle cerebral artery |
4A |
present, with collaterals filling anterior cerebral artery |
4B |
present, with no collateral circulation |
5 |
