Description

A variety of injuries to the external penis may occur in association with a wide variety of mastubatory devices.

Type of injuries:

(1) abrasions of the overlying skin

(2) lacerations into deeper tissue (urethra, corpus cavernosum, corpus spongiosum)

(3) entrapments with swelling and strangulation

(4) degloving injuries

(5) splinters or foreign material

(6) partial amputation

External masturbatory devices:

(1) vacuum devices for erectile dysfunction

(2) vacuum cleaner

(3) bottles or jars

(4) knot holes in lumber ("Dutch wife")

(5) metal or plastic washers

(6) rubber bands

(7) ball bearing ring

(8) metal or plastic pipe

Abrasions and lacerations are usually along the sides of the shaft. However, lacerations of the glans occurred in the early 1980's associated with a portable vacuum cleaner whose rotating blades were close to the mouth of the sweeper.

Issues for entrapment:

(1) not causing further harm during the removal. If a cutting device is used, to place a spatula or guard between the device and the penile skin.

(2) reducing any swelling

(3) proper lubrication

(4) Use of an appropriate cutting tool if required. A glass cutting saw may be helpful for removing glass object, while a diamond tipped dental drill may be helpful for metallic objects.

In addition to the external injuries, the possibility of internal urethral injuries should be considered if objects have been inserted into the external urethral meatus.
