Description

The TRIPICU (Transfusion Requirements in the Pediatric Intensive Care Unit) Trial restricted blood transfusion for patients in the pediatric intensive care unit (PICU) unless certain exemptions were present.


 

The threshold for a stable patient in the pediatric ICU was 7.0 g/dL.

 

Conditions allowing for blood transfusion at a higher threshold:

(1) severe acute respiratory distress syndrome (ARDS) with refractory hypoxemia

(2) shock

(3) clinical instability

(4) acute blood loss (active bleeding)

(5) surgery

(6) blood exchange-transfusion

(7) hemofiltration (if blood used to prime the machine)

(8) extracorporeal membrane oxygenation (ECMO)

(9) plasmapheresis

 

Additional exemptions used by Demaret et al:

(1) cyanotic congenital heart disease (form of refractory hypoxemia)

(2) congenital hemolytic anemia (form of acute blood loss)
