Description

An organ transplant recipient may be at increased risk for hyperinfection from Strongyloides stercoralis. The organ donor is a potential source of infection.


 

Risk factors for the organ recipient may include:

(1) failure to test a donor with risk factors for Strongyloides prior to the transplant

(2) failure to immediately notify all organ recipients after an index case is diagnosis

(3) more intensive immunosuppression

(4) masking by other conditions such as organ rejection

(4) failure to evaluate the recipient promptly

 

Exclusion:

(1) pre-existing Strongyloidiasis in the recipient that is activated following the transplant

 

Testing may include:

(1) 3 or more serial stool examinations

(2) ELISA serum testing for antibody

(3) PCR

 

Serologic testing of stored serum samples from donor and recipient taken prior to the transplant can be helpful in evaluating an infected recipient.
