Description

Haroon et al developed the Dublin Uveitis Evaluation Tool (DUET) for evaluating a patient with acute anterior uveitis. This can help to detect a patient with undiagnosed spondyloarthritis. The authors are from St. Vincent’s University Hospital and Royal Victoria Eye and Ear Hospital in Dublin, Ireland


 

Patient selection: acute anterior uveitis

 

Parameters:

(1) history of spondyloarthritis

(2) back pain with onset at age less than 45 years AND duration > 3 months

(3) joint pain requiring visit to primary care office

(4) HLA B27 status

(5) psoriasis (known or seen on physical exam)

 

If the patient has a history of spondyloarthritis then it is the presumptive cause of the uveitis and no further workup is needed.

 

The patient is referred to a rheumatologist if any of the following is positive:

(1) back pain (as above) AND HLA B27 positive

(2) joint pain (as above) AND HLA B27 positive

(3) back pain AND HLA B27 negative AND psoriasis

(4) joint pain AND HLA B27 negative AND psoriasis

 

If none of the above, then the patient is provided with a handout.
