Description

A patient who is informed about a medication and who can have his or her questions answered will tend to have more effective outcomes and fewer problems. A checklist can be used to make sure all of the pertinent information has been given to the patient.


 

General:

(1) purpose of the drug

(2) expected duration of therapy

 

Information about the drug

(1) brand and generic names

(2) visual recognition of drug and tablet strength

(3) cost

 

Mechanics:

(1) dosing and administration

(2) when to take the medication relative to meals, sleep, etc.

(3) what to do in case a dose is missed

(4) what to do if the condition being treated becomes a problem

 

Information about adverse effects

(1) recognition of minor side effects

(2) recognition of major side effects

(3) what to do if a serious complications occurs

 

Factors that may interfere with the medication or cause problems

(1) drug interactions with other prescription drugs

(2) drug interactions with over-the counter medications

(3) effects of foods on the medication and need for change in diet

 

Future care

(1) when, where and with whom followup will be provided

(2) the importance of monitoring

(3) importance to notify other health care professionals about the drug regimen
