Discrepancies in medications and instructions may occur at a patient’s hospital discharge, especially if certain risk factors are present.


Risk factors for discrepancies in medications and instructions:

(1) The provider is not familiar with the patient.

(2) The provider is not familiar with the patient’s conditions or medications (for example, a surgeon writing orders for a medical problem).

(3) There is discontinuity in care or a breakdown in communication between providers.

(4) Information in the medical record is wrong or incomplete.

(5) The provider is too busy to really check the information.

(6) The patient has many problems and medications.

 

A problem can become major if:

(1) the patient does not have a primary care provider.

(2) the discharge information is not delivered to the patient’s primary care provider in a timely fashion.
