Description

Delusional jealousy (Othello Syndrome) is a rare disorder with the patient convinced of a spouse's or companion's infidelity.


 

Features:

(1) intense jealousy often with belief of infidelity

(2) often associated with paranoia

(3) present despite contradictory evidence

 

Conditions associated with delusional jealousy:

(1) schizophrenia

(2) psychosis

(3) alcohol-related dementia or psychosis

(4) other forms of dementia

(5) Parkinson's disease with dementia or associated with dopaminergic therapy

(6) frontal lobe disorder (tumor, etc)

 

Differential diagnosis:

(1) exaggerated or pathological jealousy associated with disordered self-esteem

 

A patient with delusional jealousy may become violent and kill their spouse.
