Description

Lodise et al identified risk factors for nephrotoxicity in a patient being treated with vancomycin. These factors can help to identify a patient who may benefit from interventions to reduce the risk. The authors are from Albany College of Pharmacy, Ordway Research Institute (Albany), and Albany Medical Center.

Parameters:

(1) daily vancomycin dose in grams

(2) body weight in kilograms

(3) estimated creatinine clearance in mL per minute

(4) hospital location

total number of risk factors =

= SUM(points for all 4 risk factors)

Interpretation:

• minimum number of risk factors: 0

• maximum number of risk factors: 4

• The greater the number of risk factors the greater the risk for nephrotoxicity.
