Description

Halldorson et al reported the D-MELD score for evaluating a patient undergoing liver transplant. This can help to identify a patient who may be at increased risk for post-transplant mortality and can help guide liver allocation. The authors are from the University of Washington.


 

Patient selection: liver transplant recipient

 

Parameters:

(1) donor age in years

(2) recipient's MELD score

 

D-MELD score =

= (donor's age) * (recipient's MELD score)

 

Interpretation:

• A D-MELD score >= 1,600 is associated with a worse prognosis and longer hospital stay.

• The high risk patient has a donor age >= 60 and a recipient's MELD >= 30.

• The D-MELD score < 400 was associated with the best outcomes.
