Description

Solal-Celigny et al reported criteria for high tumor burden in patients with advanced follicular lymphomas. This can help identify a patient who may require more aggressive management. The authors are from multiple institutions in France and Belgium participating in the Groupe d'Etude des Lymphomes Folliculaires 86 Trial.


 

Criteria for high tumor burden - one or more of the following:

(1) any tumor mass (nodal or extranodal) > 7 cm in maximum diameter

(2) involvement of >= 3 extranodal sites, each with a diameter > 3.0 cm

(3) any B symptoms (unexplained fever > 38°C, drenching night sweats, unexplained weight loss > 10% from usual body weight, unexplained generalized pruritis)

(4) splenic enlargement with inferior margin extending below the umbilical line (horizontal line at the level of the umbilicus)

(5) compression syndrome secondary to tumor mass involving ureter, orbit, gastrointestinal tract or other site

(6) pleural or peritoneal serous effusion (irrespective of cell composition)

(7) circulating lymphoma cell (leukemia with > 5,000 per µL)

(8) absolute neutrophil count < 1,000 per µL

(9) platelet count < 100,000 per µL

 

where:

• The cytopenias should not be due to chemotherapy or other cause.
