Description

Celiac crisis is a rare but serious manifestation of celiac disease.


 

Patient selection: celiac disease

 

Criteria for celiac crisis:

(1) acute onset or rapid progression of gastrointestinal symptoms secondary to celiac disease

(2) requires hospitalization and/or parenteral nutrition

(3) >= 2 clinical and laboratory findings (see below)

 

Clinical and laboratory findings:

(1) severe acute diarrhea with dehydration

(2) abnormal electrolyes (hypokalemia, hypocalcemia, hypomagnesemia, hyper or hyponatremia)

(3) renal dysfunction with serum creatinine > 2 mg/dL

(4) metabolic acidosis with pH < 7.35

(5) hypoalbuminemia (serum albumin < 3.0 g/dL)

(6) weight loss > 10 lbs (4.5 kg)

(7) neurologic dysfunction including paralysis

 

The diagnosis requires exclusion of other explanations for the acute deterioration.
