Collagenous gastritis is a rare form of chronic gastritis.
Key histologic criteria:
(1) superficial subepithelial collagen layer > 10 microns thick
(2) inflammatory infiltrate in the lamina propria
The collagen layer can be demonstrated with:
(1) trichrome stain
(2) immunostain for tenascin
Distinct histologic patterns:
(1) lymphocytic gastritis with > 25 lymphocytes per 100 surface enterocytes
(2) eosinophil-rich (> 30 eosinophils per high power field)
(3) atrophic
Clinical associations:
(1) celiac sprue
(2) collagenous colitis
(3) collagenous sprue
(4) rarely IgG4-related disease
