Description

Mont and Maar classified fractures of a femur that also treated with a total hip prosthesis. The type of fracture can help determine the optimum management strategy. The authors are from Good Samaritan Hospital in Baltimore.

Type

Description

1

intertrochanteric

2

in proximal femur (subtrochanteric or diaphysis, spanning the stem of the prosthesis)

3

spanning the distal tip of the prosthesis

4

distal to the distal tip of the prosthesis

5

comminuted

6

supracondylar (distal femur)

Type

Management Suggestion

1

nonoperative (traction and casting)

2

cerclage fixation and bone-graft, OR revision to a cemented long-stem prosthesis with cerclage fixation and bone graft

3

cerclage fixation and bone-graft, OR revision to a cemented long-stem prosthesis with cerclage fixation and bone graft

4

long-stem revision and simple traction

5

long-stem revision with supplementary fixation and bone-graft
