Clinical Features

Class

scalp wound without skull fracture

I

skull fracture without dural penetration

II

compound skull fracture with dura and brain penetrated

III

Class III with complicating factor

IV

 

Subtypes of Class III:

(1) gutter type with no retained missile

(2) penetrating type with missile retained in the brain

(3) perforating with no retained missile (through-and-through)

 

Complicating factors for Class IV:

(1) ventricular penetration

(2) fracture of the orbit or sinus

(3) injury to the dural sinus

(4) intracerebral hematoma
