Grade

normal mucosa

0

erythema or friability of mucosa without macroscopic erosions

1

superficial ulceration or erosions involving less than 10% of mucosal surface area in the last 5 cm of esophageal squamous mucosa

2

superficial ulceration or erosions involving 10-49% of mucosal surface area in the last 5 cm of esophageal squamous mucosa

3

superficial ulceration or erosions >= 50% of mucosal surface area with or without confluence

4

deep ulceration anywhere in the esophagus

In the modified version in McDonagh et al Grade 5 was a stricture (narrowing that prevents easy passage of the endoscope).
