The Blue Liver Syndrome (BLS) can occur following chemotherapy and refers to the gross appearance of the liver at surgery.
Features of the blue liver syndrome:
(1) The patient has a history of chemotherapy with a drug associated with sinusoidal obstruction (oxaliplatin, other).
(2) The liver appears blue at surgery.
(3) Microscopic examination of the liver shows sinusoidal obstruction and congestion.

• Some of the reports are from patients who underwent preoperative indocyanine green retention studies. One of the risk factors for BLS was a high retention rate. It might be interesting if the dye contributed to the gross color of the liver.
