Description

A physically active person or athlete with hypertension can exercise or compete provided that he or she manages the disease appropriately.


 

Discontinue:

(1) alcohol, drug or stimulant abuse

(2) medications contributing to an elevation in blood pressure

 

Activities to avoid (due to the risk for a sudden rise in blood pressure):

(1) weight lifting and body building

(2) isometric or intense resistive exercise

alpha-adrenergic blocker, not centrally acting

hypotension with first dose or abrupt exercise cessation

angiotensin converting enzyme (ACE) inhibitor

avoid pregnancy if female (teratogenic)

dry cough

angiotensin receptor blocking (ARB) agent

avoid pregnancy if female (teratogenic)

beta-adrenergic blocker

banned by sports organizations

decreased peak performance

problems in regulating body temperature in the heat

may provoke hypoglycemia

exercise-induced asthma or bronchoconstriction

calcium channel blocker (CCB), dihydropyridine

hypotension with abrupt exercise cessation

fluid retention

vascular headaches

reflex tachycardia

calcium channel blocker (CCB), other

hypotension with abrupt exercise cessation

suppressed heart rate

constipation

thiazide diuretic

banned by sports organizations

problems in regulating body temperature in the heat

may provoke hypoglycemia

hypovolemia with orthostatic hypotension

hypokalemia and hypomagnesemia

 

Drugs not recommended because of side effects:

(1) loop diuretics

(2) centrally-acting alpha blockers
