Description

Lindstedt et al evaluating a protocol for antibiotic prophylaxis prior to core biopsy of the prostate. The authors are from Malmo University Hospital, Lund University Hospital and Lund University in Sweden.

Patient selection: male undergoing ultrasound-guided core biopsy of the prostate

Regimen: One oral dose of ciprofloxacin 750 mg just prior to the procedure or up to 2 hours before..

Contraindications:

(1) hypersensitivity to ciprofloxacin

(2) artificial heart valve

(3) bacteriuria prior to the procedure

Potential risk factors for infection associated with the core biopsies:

(1) indwelling urinary catheter for acute urinary retention (bladder outlet obstruction)

(2) history of repeated urinary tract infections

(3) prostatitis

(4) current steroid therapy

(5) repeat prostate biopsy

If the patient has no risk factors then the risk of infection was about 1%. The presence of a risk factor increased the risk of infection, and major problems can occur in patients with an unrecognized urinary tract infection at the time of the biopsy.
