Description

Anascorp is an antivenom to scorpion venom that has been approved for severe scorpion envenomations.


 

Anascorp consists of antibodies prepared from horse (equine) serum with removal of Fc portion, resulting in F(ab)2 fragments.

 

Patient selection:

(1) severe scorpion envenomation (tongue fasciculations, uncontrolled limb jerking, opsoclonus, hypersalivation, etc)

(2) no history of allergy to Anascorp or horse serum

 

Initial management:

(1) Start an intravenous catheter.

(2) Administer oxygen.

(3) Place on a cardiac monitor.

(4) Provide pain control if needed.

(5) Provide sedation if agitated.

 

Dosing of Anascorp:

(1) initial: 3 vials infused over 10 minutes

(2) single vial infused over 10 minutes if symptoms not controlled in 30 minutes

(3) single vial infused over 10 minutes if symptoms not controlled after an additional 30 minutes

 

Reconsitute each vial in 5 mL of sterile normal saline. Add suspension to 50 mL infusion bag of sterile normal saline.

 

If symptoms are controlled after administration of Anascorp, then continue to monitor the patient. No further therapy is needed.

 

If symptoms persist after 5 vials, then:

(1) continue to monitor the patient.

(1) consider the presence of other conditions.

(2) transfer the patient.
