Description

Zhong et al used changes in the electrocardiogram to distinguish atrioventricular node re-entrant tachycardia (AVNRT) from atrioventricular reciprocating tachycardia (AVRT) with a concealed accessory pathway. The authors are from Peking University People’s Hospital in Beijing.


 

Features of atrioventricular node re-entrant tachycardia (AVNRT):

(1) pseudo r wave in V1

(2) S/Q wave in inferior leads

(3) absence of ST elevation in aVR (see below)

 

Features of atrioventicular reciprocating tachycardia (AVRT)

(1) RP interval >= 100 ms

(2) ST elevation at 80 ms after J point in aVR (>= 1.0 mm if horizontal or upsloping OR >= 1.5 mm if downsloping)

 

Performance:

• The algorithm correctly diagnosed the mechanism of the tachycardia in about 85%.
