Description

Jeong et al used a flow diagram to triage a specimen for dilution based on the availability of past data. This can help to expedite testing, especially for stat requests. The authors are from the University of Ulsan in Seoul, Korea.

Assay: serum alpha-fetoprotein (AFP)

Cutoff for automated dilution: 200 ng/mL

Cutoff for manual dilution: > 5,000 ng/mL

Parameters:

(1) history of past AFP results

(2) current assay result

The specimen is reported directly if:

(1) there is no history of a previous AFP AND the current result is <= 200 ng/mL

(2) the previous AFP was <= 200 ng/mL AND the current result is <= 200 ng/mL

The specimen is sent for automated dilution if:

(1) there is no history of previous AFP AND the current result is > 200 ng/mL

(2) the previous AFP was <= 200 ng/mL AND the current result is > 200 ng/mL

(3) the previous AFP was > 200 and <= 5,000 ng/mL

The specimen is sent for manual dilution if:

(1) the previous AFP was > 5,000 ng/mL

(2) a result with automated dilution is > 5,000 ng/mL
