Intravenous infusion of an aminobisphosphonate (such as zoledronic acid or pamidronate) can result in an acute phase reaction.
The reaction tends to occur during the first infusion.
Clinical findings consist of flu-like symptoms:
(1) fever
(2) headache
(3) myalgias
(4) arthralgias
(5) chills
(6) fatigue
(7) nausea
Laboratory findings:
(1) elevated serum levels of cytokines (TNF, interferon, IL-6)
The symptoms peak about 28-36 hours after the infusion.
The reactions can often be prevented by administration of ibuprofen or acetaminophen.
Differential diagnosis:
(1) influenza or other viral infection
(2) other drug reactions with release of cytokines
